Social Worker / Care Case Manager

Always Here Home Care is a privately owned, boutique home care agency that prides itself on caregiving excellence.  We are looking for an experienced and compassionate Care Case Manager (Social Worker). The Care Case Manager will coordinate care between the client or client representatives (family, GCMs, nurses or facility representatives) and our caregivers. 

Always Here's caregivers provide non-medical care to the elderly and disabled with a focus on hospice, companionship, personal care and homemaker service.  

Position Responsibilities

  • Perform client intake and assessment (communication, paperwork collection, and data entry)
  • Create/maintain client files (paper and electronic)
  • Use assessment to create client-specific care plans; share findings and planned actions with client representatives and office staff to facilitate a collaborative approach to meet each client’s needs
  • Coordinate all aspects of the clients’ care
  • Collaborate with Care Case Manager and Scheduler to ensure consistent schedule with caregivers who possess the skill set to meet the clients’ needs
  • Facilitate client access to services in the community, through timely initial and ongoing assessments
  • Disseminate pertinent client-related information to caregivers
  • Provide education and training to caregivers
  • Provide crisis management
  • Conduct client visits, including initial client/caregiver introductory visit
  • Maintain upkeep and delivery of client postings (logs/schedules/postings)
  • Provide input on caregiver performance and behavior to office staff and Human Resources if and when needed
  • Draft caregiver profiles
  • Schedule or fill emergency shifts
  • Document all communications with clients, client representatives, facilities, geriatric care managers, caregivers, etc
  • Perform other related duties as required and assigned
    • Work extended hours and occasional weekends, as required
    • After-hours and weekend phone coverage, one a month (additional pay and only back-up to on-call manager)
    • Possible caregiver recruitment, upon reques
    • Fill-in shifts, as required
    • Special projects, as requested
